According to WHO, Indonesia ranks fourth in the largest number of patients with Diabetes Mellitus in the world. Data in 2000 stated that there were approximately 5.6 million people in Indonesia who have diabetes. In 2009, this number reached 8 million people and is predicted to increase to more than 21 million people in 2025. This fact makes Indonesia ranks fourth as the country with the highest number of diabetics in the world.

What is Diabetes ?

Diabetes which is also known in Indonesia by the term Kencing Manis is a metabolic disorder with symptoms such as high ratio of sugar in the blood and impaired metabolism of carbohydrates, fats and proteins.

In the case of diabetes, glucose (simple sugar) level in the blood is high because the body can not release or use insulin adequately.

When a person has diabetes then,

  1. The pancreas cannot produce enough insulin to absorb sugar from food. It causes blood sugar level rises due to deposition of sugar from food that cannot be absorbed properly and burned into energy.

  2. Another cause is defective insulin or the body cannot use insulin properly.
Insulin in Pancreas

Insulin is a hormone produced by the pancreas, an organ next to the flank. Insulin works to open the receptors on the cell walls so that glucose enters the cells. Then the cells convert glucose into energy needed by the body to perform the activity.

The causes of diabetes depend on the type of diabetes suffered.

There are 2 types of diabetes.
The most common and affects many people are diabetes type 1 and type 2.

Type 1 diabetes is often called Insulin Dependent Diabetes Mellitus (IDDM) or a Dependent Diabetes Mellitus on Insulin. Type 1 diabetes is associated with the inability of the pancreas to produce insulin. So, this type of diabetes is often associated with the damage or impaired function of the pancreas to produce insulin. In type 1 diabetes, the pancreas cannot produce enough insulin because the body's immune system disorders that destroy cell that produce insulin, or due to virus infection so that the insulin hormone in the body is reduced and resulting in accumulation of sugar in the bloodstream.

Causes of Type 1 Diabetes
Some causes the pancreas cannot produce enough insulin in people with type 1 diabetes are:
  • Heredity or genetics. If one or both parents suffer from diabetes, then the child would be at risk of developing diabetes.
  • Autoimmunity is when the body is allergic to one type of tissue or cell itself - in this case, is in the pancreas. The body loses its ability to form insulin because the body's immune system destroys the cells that produce insulin.
  • Virusor chemicals that cause damage to the cell islands (groups of cells) in the pancreas where insulin is made. The more cell islands damaged, the more likely a person suffers from diabetes.

Type 2 diabetes is also often called Non-Insulin Dependent Diabetes Mellitus or Diabetes Mellitus Without Depending on Insulin. In type 2 diabetes, the problem occurs because the insulin produced is insufficient. Most of the insulin produced is absorbed by fat cells because of unhealthy style and diet. While the pancreas cannot produce enough insulin to overcome insulin deficiency, the level of blood sugar will rise.

Cause of Type 2 Diabetes

Some of the main causes of type 2 diabetes can be summarized as follows :

  • Heredity, if a parent or a sibling has it.
  • A diet or an unhealthy lifestyle The number of fast food outlets (fast food) serving fatty and unhealthy foods.
  • A High cholesterol level.
  • Occasional exercise.
  • Obesity or overweight.
All causes of type 2 diabetes are generally caused by an unhealthy lifestyle. This creates imperfect metabolism in the body that leads to the improper function of insulin in the body. The insulin hormone can be absorbed by the fat in the body.

The difference
between type 1 and type 2 diabetes are if :
Type 1 diabetes is caused by the inability of the pancreas to produce insulin, while type 2 diabetes is caused by lack of insulin due to the improper work of the pancreas. while type 2 diabetes is caused by lack of insulin due to the improper work of the pancreas..

Potential complications of diabetes

Stroke :
Causing loss of neurological function, causing numbness, weakness in speech, body coordination, or walking.

Eye disease :
Causing blind spots or blindness.

Heart Disease :
Causing heart attack and congestive heart failure.

Kidney Disease :
Leading to kidney failure.

Circulatory problems :
Causing improper heal of wounds. In extreme situations, gangrene can occur and can lead to amputation.

Gangrene is a wound that had rotten and it can spread. It is characterized by the dead tissue which turns blackish in color and smells because of the decay by the bacteria.

In general, some of the symptoms that occur include :

  • Frequent urination.
  • Frequent thirst.
  • Frequent hunger because of insufficient supply of energy, so the body signals hunger.
  • A sudden weight loss although there is no attempt to lose weight. It happens because when the body cannot distribute the sugar into the cells, the body burns its own fat and protein for energy.
  • Frequent tingling in the feet or hands.
  • Experiencing skin problems like itching or skin ulcers.
  • If injured, it takes a long time to heal.
  • Changes in behavior such as irritability. It happens because type 1 diabetics often wake up at night to urinate so they cannot sleep well.
  • Easy to feel tired

What is the normal blood sugar level?

Sugar levels after fasting :

  • Normal: below 100 mg/dl
  • Pre-diabetes: 100 - 126 mg/dl
  • Diabetes: above 126 mg/dl

Sugar levels two hours after eating :

  • Pre-diabetes: 140 - 200 mg/dl
  • Diabetes: above 200 mg/dl
